Employee Benefits

Midland Health offers several programs that support your personal and professional goals, plus a variety of medical, dental and other benefit options that provide the coverage that best suits you.

Health and Wellness

Health Insurance/Medical Coverage

Midland Health offers a High Deductible Health Plan (HDHP) with a Health Savings Account (HSA) to help you cover a wide range of medical services. These services include office visits, hospitalization, labs, x-rays, and prescription drugs. An HDHP is a consumer-driven health plan. This plan has a higher deductible than a traditional plan, which then encourages the employee to take a more active role in their health care decisions. The plan still allows you to use the physician or health care provider of your choice; however, you will receive the highest level of benefits when you use Midland Health and other in-network providers.

Dental

Midland Health recognizes that healthy teeth and regular oral check-ups are an important part of your overall well-being. For this reason, we provide dental insurance that encourages preventive care and regular exams. Additionally, Midland Health’s dental plan allows you to visit any licensed dental provider in the United States. Guardian PPO covers 100% of preventive services such as oral exams and routine cleanings. It also covers 80% of basic services such as X-rays and fillings, 50% of major services such as root canals and crowns, and 50% of orthodontics for dependent children.

Vision

If you wear glasses or contacts, chances are you already have ongoing appointments with an eye doctor. However, even those with perfect eyesight should have their vision checked on a regular basis. To ensure that you and your family have access to quality vision care, Midland Health offers comprehensive vision benefits through Superior Vision Network. Coverage includes an annual exam and either contact lenses or eyeglass frames once every 12 months.

Prescription Drugs

When you choose to enroll in the medical plan, you automatically have prescription drug coverage. Your prescription cost will go towards your deductible. Once your deductible has been met then you will have a coinsurance percentage that will apply per prescription order or refill. Your lowest cost option will always be the use of generic drugs whenever possible. Midland Health also has an Outpatient Pharmacy that is available for employees.

BetterU Online Wellness Program

Focusing on your health can pay off. Midland Health offers an employee wellness program, as well as an online wellness portal called Better U. Employees may be eligible to receive Health Savings Account dollars based on participation in the wellness program.

Employee Gym

We have an on-site gym and showers that are available for employees.

Health & Wellness Clinic

Midland Health cares about your wellbeing. It can be stressful juggling your loved ones or your own sickness with the expectation of work. Midland Health employees and covered dependents have access to a clinic with minimal waiting times and negotiated rates. A fee of $50 per visit covers the full cost of most services provided, including office visits and routine labs.

Lifestyle Medicine Center

At Midland Health, we believe that one cannot be truly healthy without considering the mind, body, and spirit. Because of this, we created the Lifestyle Medicine Center—a resource for the community to provide physical, mental, spiritual, and financial education so that people can experience the best life possible. Providing holistic health resources for the community is one more example of how Midland Health is helping to make Midland the healthiest community in Texas. The Lifestyle Medicine Center offers the following programs:

Financial Wellness

Retirement Savings Plan

Midland Health offers a 403(b) plan. Upon employment, you are immediately eligible to participate in the plan. Midland Health provides a matching contribution for full-time and part-time employees based on years of service. PRN employees may contribute to their 403(b), but are not eligible for the employer match.

  • If you have less than 5 years of service, you will receive a maximum matching contribution of 50% on the first 6% deferred.
  • If you have between 5 and 9 years of service, you will receive a maximum matching contribution of 75% on the first 6% deferred.
  • If you have between 10 and 19 years of service, you will receive a maximum matching contribution of 100% on the first 6% deferred.
  • If you have more than 20 years of service, you will receive a maximum matching contribution of 100% on the first 7.5% deferred.

Health Savings Account

Take charge of your healthcare spending with a Health Savings Account (HSA). Contributions to an HSA are taken on a pre-tax basis and are tax-free when used for eligible medical expenses, including those of your spouse and/or dependent(s), even if they are not covered by your Plan. Employees may also be eligible for an HSA employer contribution, depending on their hourly rate.

Short-term and Long-term Disability

If an employee is unable to work due to an illness or injury that is not work-related, the short-term disability (STD) plan provides benefits after 14 calendar days. This benefit is payable for up to 11 weeks. Eligible employees who continue to be disabled after 90 days may be eligible to receive long-term disability (LTD) benefits.

Basic Life Insurance and Accidental Death and Dismemberment (AD&D) Insurance

Life Insurance and AD&D benefits are essential to the financial security of you and your family. Midland Health provides employees with Basic Life and AD&D insurance through Lincoln Financial Group. This coverage will provide financial support to loved ones, such as a spouse or other designated survivor(s) after an employee’s death. Your Basic Life and AD&D insurance benefit is equal to 1 time your annual salary. Midland Health provides these two benefits at no cost to the employee. However, employees may elect to purchase additional life insurance coverage if they wish.
