Self-Schedule Your COVID-19 Vaccination for Adults and Children

Please make sure you click the link that is appropriate for your age range to schedule. 

Notice: A child must be accompanied with a parent or guardian and MUST have the following two forms filled out and taken to the appointment: Registration Form. Screening Form.
If you arrive without these forms completed, your appointment may be rescheduled. In addition, parents/guardians must read the required reading document by clicking here.
