Midland Unified Command Team to Transition to one Mass Vaccination Site

Effective Monday, March 8, the Midland Unified Command Team will close the Golf Course Road Church of Christ Mass Vaccination Site. The Midland County Horseshoe Backstage location, 2514 Arena Trail, will remain open for COVID-19 first and second dose administration.

If you received your first dose of the COVID-19 vaccine at the Golf Course Road Church of Christ location between February 16 and March 5, you will receive your second dose on your scheduled date and time at the Horseshoe Backstage location.

If you meet criteria to receive the vaccine, you can self-schedule your first dose vaccine at midlandhealth.org/schedulemyvaccine. You can also register to get on the list by visiting midlandhealth.org/vaccine and clicking on the Click to Register button. Detailed information defining the populations included in these phases is available on the above-mentioned website.

Once your vaccine is scheduled, you will receive an e-mail, text message and/or phone call with the information needed.  This communication will come from support@healthipass.com, 855-897-1447(text), or 432-221-1111(phone call). 

After you have a confirmed date/time of your vaccine, make sure you complete all forms online that are sent to you prior to arriving for the vaccine. If you do not complete the registration process prior to arrival, there may be a delay in your vaccination. Please take a valid picture ID with you to your appointment, arrive on time and wear a short sleeve or sleeveless shirt.  If a jacket is needed make sure it is easy to remove. 

If you are interested in volunteering at the mass vaccination sites, e-mail vaxvolunteers@midlandhealth.org, call 432-312-0030, or text 432-242-2149.  Please include your name, phone number, clinical/non-clinical and what days/times you are available over the next six weeks between 8:45am - 5:00pm. We are offering our vaccine site volunteers a COVID-19 vaccine.
