Employee Benefits
Midland Health offers several programs that support your personal and professional goals, plus a variety of medical, dental and other benefit options that provide the coverage that best suits you.
Health and Wellness
Health Insurance/Medical Coverage
Midland Health offers a High Deductible Health Plan (HDHP) with a Health Savings Account (HSA) to help you cover a wide range of medical services. These services include office visits, hospitalization, labs, x-rays, and prescription drugs. An HDHP is a consumer-driven health plan. This plan has a higher deductible than a traditional plan, which then encourages the employee to take a more active role in their health care decisions. The plan still allows you to use the physician or health care provider of your choice; however, you will receive the highest level of benefits when you use Midland Health and other in-network providers.
Dental
Midland Health recognizes that healthy teeth and regular oral check-ups are an important part of your overall well-being. We provide dental insurance that encourages preventive care and regular exams. Additionally, Midland Health’s dental plan allows you to visit any licensed dental provider in the United States. Guardian PPO covers 100% of preventive services such as oral exams and routine cleanings. It also covers 80% of basic services such as X-rays and fillings, 50% of major services such as root canals and crowns, and 50% of orthodontics for dependent children.
Vision
If you wear glasses or contacts, chances are you already have ongoing appointments with an eye doctor. However, even those with perfect eyesight should have their vision checked on a regular basis. To ensure that you and your family have access to quality vision care, Midland Health offers comprehensive vision benefits through Superior Vision Network. Coverage includes an annual exam and either contact lenses or eyeglass frames once every 12 months.
Prescription Drug
When you choose to enroll in the medical plan, you automatically have prescription drug coverage. Your prescription cost will go towards your deductible. Once your deductible has been met then you will have a coinsurance percentage that will apply per prescription order or refill. Your lowest cost option will always be the use of generic drugs whenever possible. Midland Health also has an Outpatient Pharmacy that is available for employees.
Better U Wellness Program
Focusing on your health can pay off. Midland Health offers a unique, online wellness program called Better U. Employees are eligible to receive Health Savings Account dollars for participating in this wellness program.
Employee Gym
We have an on-site gym and showers that are available for employees.

Midland Health cares about your wellbeing. It can be stressful juggling your loved ones or your own sickness with the expectation of work. Employees may also be eligible for an HSA employer contribution, depending on their hourly pay.
Short-term and Long-term Disability
If an employee is unable to work due to an illness or injury that is not work-related, the short-term disability (STD) plan provides benefits after 14 calendar days. This benefit is payable for up to 11 weeks. Eligible employees who continue to be disabled after 90 days may be eligible to receive long-term disability (LTD) benefits.
Basic Life Insurance and Accidental Death and Dismemberment (AD&D)
Life Insurance and AD&D benefits are essential to the financial security of you and your family. Midland Health provides employees with Basic Life and AD&D insurance through Lincoln Financial Group. This coverage will provide financial support to loved ones, such as a spouse or other designated survivor(s) after an employee’s death. Your Basic Life and AD&D insurance benefit is equal to 1 time your annual salary. Midland Health provides these two benefits at no cost to the employee. However, employees may elect to purchase additional life insurance coverage if they wish.
