DAISY Team Award Nomination
The DAISY Team Award honors nurse-led groups of two or more individuals who go above and beyond in patient care. This award celebrates collaboration, compassionate care, and positive outcomes for patients and their families.
AWARD CRITERIA
To be considered for the DAISY Team Award, the team must meet the following criteria:
Nurse-led: The team must be led or co-led by a nurse.
Collaboration: Team members must work together across roles, departments, or disciplines.
Meaningful Impact: The team must demonstrate a positive impact on patients, families, colleagues, or the community through extraordinary teamwork.
What winners receive: Winning teams receive a customized, engraved DAISY Team Award plaque, special DAISY Team pins for each team member, and a celebratory presentation that often includes cinnamon rolls, a DAISY tradition.
