It is with considerable mixed emotions that I am announcing that I will be leaving my practice with Premier Physicians, Department of Rheumatology effective September 2024. It has been both my privilege and my pleasure to have been able to provide Rheumatology care to the residents of West Texas while working under both Premier Physicians and Midland Health.

I will be establishing an office under the name of Basin Rheumatology which will be located at 3409 Andrews Hwy Suite B. Midland Tx. 79703 and will start seeing patients effective immediately. To schedule a follow up appointment please call 432.599.3556 Monday- Thursday, 8am- 5pm.

If you choose to continue to seek care with me, you will need to sign an authorization form before Premier Physicians, Department of Rheumatology can release your medical records. Please note, by law we cannot share your medical information without your written consent.  You may sign this form at our office.

Your medical records are confidential and will remain on file with Premier Physicians, Department of Rheumatology. If you choose to establish your care with Dr. Bellam, you do not need to take any action concerning your records.

It has been my great pleasure to care for you as my patient and I look forward to continuing to serve Rheumatology needs in the Permian Basin in the years to come.
